Well we do that in November as remembering the people who passed away, people who were close to us.
Christmas Eve is one of the most important holiday, celebration in Poland, and it reminds us of people we used to share it with. So as the symbol that we still remember and keep them in our hearts we go and put the candles and flowers also on Christmas Eve.
We visit family graves quite often here in Poland. Most people clean them and decorate them for every major holiday, not just All Souls Day. And its common here to take care of them and light candles often, not just around the holidays. If you thought they looked beautiful around Christmas time then you should see them in November:)
On Christmas and All Saints Day
All Saints Day (Zaduszki, Wszystkich Świętych), 1st of November. In afternoon and evening people visit graves of their relatives and light candles. After dusk cemeteries glow with thousands of lights and offer a very picturesque scene. For the most part, stores and restaurants will close, although, not to the extent on Christmas Eve. You should be able to find a few restaurants, if you’re persistent. If you have the chance, be sure to visit a cemetery, although you may have to wait for public transportation as services are drastically cut.

There is nothing special about this and no real celebration except that it is Christmas and we remember our loved ones when our family members get together from different towns, and that is why you will see a concentration of candles and flowers on graves.
As other posters have said the real celebration of all saints day is in November, but really you will cemeteries lit up all through the year, as Polish people really look after the graves of their departed ones.
I know one old woman who every day goes to clean her husbands grave, “so sweet”!!
I know of another story where a husband purchased a grave plot for his wife as a Christmas present, and she was really happy about it because it was next to her mother and father.
